Client Coaching Agreement

Please Read Agreement. Once you proceed, it is implied that you agree to the following conditions. 
By investing in this membership program you are agreeing to the following Coaching Client Agreement. A copy of this agreement will be provided for you upon accessing your membership area as well.

This agreement is entered into between Path to Health and Healing, LLC and you the client. Drawing on my background, training, skills and life experiences, I support my clients—spiritually, mentally, emotionally and physically. I am a holistic and functional nurse practitioner. Consulting with me inside this membership is not meant to replace seeking appropriate medical care nor will medical advice be given. I will be working with you strictly in a coaching capacity.

This content is not intended to diagnose or treat any diseases. It is intended for informational, educational and self-empowerment purposes ONLY. Please consult your primary healthcare provider before making decisions based on what is best for your unique genetics, culture, health conditions and stage of life.

Coaching Agreement: 
By joining the Path to Hormone Health Membership Program, you are entering into an agreement with Path to Health and Healing, LLC. It will begin on date of purchase and will continue until you cancel your membership.  A 3-month minimum commitment in this membership is required. You can cancel your membership at any time thereafter.  

You will have access to the Path to Hormone Health Membership for as long as you remain an active member. You will be given access to our private Community Discussion Board and Facebook group.  

As part of your membership, you will receive the Path to Hormone Health Program will delivered to you one module per month.  There are 12 modules.  So if you cancel before the end of 12 months, you will have access to the number of months you are a member.    

All other programs and bonus offerings included with your membership will be delivered as they become available.  

Fees: The fees for this membership were agreed upon at the time you enrolled and at time of purchase of your Path to Hormone Health Membership.
 
Monthly Plan: Initial month will be charged at time of enrollment.  One month later, you will receive an invoice from Paypal for your recurring payments .  You may cancel your membership anytime after your initial 3 month commitment.      

Annual Plan:  After your initial annual payment, you will be sent an invoice from Paypal on your annual renewal date as long as you choose to remain an active member.  You may cancel at any time.  

As noted, payments will auto-renew monthly or if you purchased annual membership plan, on a yearly basis.  

Cancellation Policy: As with most digital and print offerings from video, audio and pdf format, there are generally no refunds on programs or products that can be downloaded, viewed, copied or stored in an electronic format. This is an industry standard. You may cancel any time after your 3-month minimum commitment either by cancelling your payments or by notifying Path to Health and Healing at laura@pathtohealthandhealing.com. No partial month refund will be given.  

The information contained within this membership platform is for your personal use, and you cannot sell to, share or exchange any of this material with any other health and wellness professional, customer, client, or friend. This material is strictly for your personal use and benefit; therefore, no part of it can be used in any other manner including but not limited to reselling, copying or sharing of information.

 All materials are to remain the property of their respective owners. Materials made available to the private group forums by email, or any other means may not be distributed in any fashion, print or electronic, without expressed written permission from Path to Health and Healing, LLC. Thank you for your understanding.

Services: Access to private Community Board and Facebook or other video conferencing software at the coach’s discretion. You will be provided with information on how to ask to join these private groups for approval.  

The purpose of the group is to provide the client with information based on their defined goals and questions or experiences to share within the group and to offer support. We all learn from each other. This service includes but is not limited to: identifying actions steps, follow-up on how members of the group are doing and to help maintain client motivation, educating the client as well as asking and answering questions.

Termination: Termination of this agreement may occur at the discretion of the coach when an impasse is reached with the client. As long as the client adheres to the Membership Guidelines and the coach adheres to professional practices and standards, the relationship will continue. The goal is to better your hormone health, so all efforts will be made towards that end.

Examples of terminable offenses:
• Lack of payment
• Lack of adherence to client agreement and group Guidelines
• Indifference on the part of the client to prescribed session action items
• Inappropriate behavior that is offensive to members of the group or the coach

Confidentiality: Anything that is said or revealed in the groups coaching sessions is between the coach and members of the group. This is privileged information and will not be disclosed to any outside party. The exception is if the information revealed includes disclosure of illegal, unethical or criminal activities.

The methods employed by Path to Health and Healing, LLC do not guarantee results. They are guidelines that the client must follow through on in order to see any type of change. The client agrees, therefore, to hold the coach free of any liability and responsibility for adverse reactions and outcomes as a result of advice given in the coaching sessions and within the program materials.

By your purchase it is implied that you agree with information laid out in this client agreement in full.
